The Connecticut Home Care Program for Elders, administered by the Department of Social Services, enables individuals aged 65 and older to remain safely in their home with the proper supports and services instead of residing and receiving care in a nursing facility or other institutional setting.

The types of care and services required depend upon how much assistance is needed with your Activities of Daily Living (ADLs). These are the activities that are essential to day-to-day functioning such as: bathing, dressing, grooming, toileting and transferring.  Assistance may also include tasks such as home chores, shopping, laundry, medication reminders, companionship and other activities that are essential for independence.

Administered by the Connecticut Department of Social Services, the Personal Care Attendant Waiver program enables eligible adults aged 18 - 64 years with long-term care supports and services to live independently at home and to engage with their communities. The goal of the PCA program is to provide an alternative to entering an institution and to have greater independence in a community setting.

Adult Family Living is offered as a unique home care program under the Connecticut Home Care Program for Elders and the Personal Care Assistance Waiver.  Adult Family Living allows older adults to live in a private home and receive care from a family member or friend who is paid a weekly stipend for their caregiving work.

Personal care and other support services are provided to the older adult by a non-spouse caregiver who is typically a family member or friend who lives in the home that is owned by either the older adult or the caregiver.

The Money Follows the Person Program is administered by the Connecticut Department of Social Services and is a state and federally funded program that enables Medicaid recipients living in long term care facilities such as nursing homes to move back to their communities. Individuals are assessed for eligibility for a variety of Medicaid waiver programs then the available services and supports are established into an appropriate Plan of Care.

Access to funding through these waiver programs allows the individual transitioning to community living the opportunity to live safely and happily with long-term supports and services equal to those they would receive in a long-term care facility. Program participants also have access to housing subsidies and supports not available to other programs such as home modifications and Assistive Technology.

DSS Adult Protective Services:  The Adult Protective Services Program administered by the Connecticut Department of Social Services is designed to secure adults (18 - 59 years old) from various forms of maltreatment and abuse such as physical, mental and emotional abuse, neglect, abandonment, and/or financial abuse and exploitation. This includes allegations of abuse or neglect of residents in long-term care facilities. Generally, the types of abuse involve: physical, emotional, sexual, neglect, abandonment and financial exploitation.

The program provides emergency Personal Care Assistance, Homemaking and Companion services to individuals 18 to 59 years of age who are determined to be suffering from abuse, neglect or abandonment in order to allow these individuals to remain living in the community with long-term care services and supports.

The Protective Services for the Elderly Program is administered by the Connecticut Department of Social Services and is designed to safeguard people 60 years and older from physical, mental and emotional abuse, neglect, abandonment, and/or financial abuse and exploitation. This includes allegations of abuse or neglect of residents in long-term care facilities.  Generally, the types of abuse involve: physical, emotional, sexual, neglect, abandonment and financial exploitation.

The program provides emergency Personal Care Assistance, Homemaking and Companion services to individuals 60 years and older who are determined to be suffering from abuse, neglect or abandonment in order to allow these individuals to remain living in the community with long-term care services and supports.

The Veterans Administration's Community Care Network ensures that our veteran beneficiaries receive timely and quality care that follows the standard guidelines of the US Department of Veteran Affairs. Participants must be eligible for Veteran’s Administration benefits and require assistance with Activities of Daily Living (ADLs) or Instrumental Activities of Daily Living (IADLs) and will be provided Personal Care Assistance, Homemaking, and Companion services based on their individual needs.

Assistance under the program can include: Bathing, Dressing, Grooming, Toileting, Transferring, Light housekeeping, Companionship, Laundry and more.

The Veterans Administration's Aid and Attendance Benefit program offers a monthly pension for qualified veterans and survivors. To be eligible as a beneficiary, applicants have to need help from caregivers or another person in fulfilling their daily activities.

You may be eligible for this benefit if you get a Veteran’s Administration pension and you meet at least one of these requirements:

• You need another person to help you perform daily activities, like bathing, feeding, and dressing, or
• You have to stay in bed—or spend a large portion of the day in bed—because of illness, or
• You reside in a nursing home due to the loss of mental or physical abilities related to a disability, or
• Your eyesight is limited (even with glasses or contact lenses you have only 5/200 or less in both eyes; or concentric contraction of the visual field to 5 degrees or less)

Community First Choice, administered by the Connecticut Department of Social Services, is a Medicaid benefit for people who are at an institutional level of care. This means that if you qualify for care in a nursing home or other institutional setting, under your Medicaid benefit, you also qualify for CFC services in your home and community to assist with Activities of Daily Living (ADL), Instrumental Activities of Daily living (IADL), and Health-related tasks.

CFC is a self-directed program which is when you, or someone you appoint, make the decisions regarding your care and services. You have control over what services you want in the home, and you have the responsibility of managing those services. Self-direction promotes personal choice and control during a person-centered planning process.

Private Pay Care Services can include Personal Assistance, Homemaking, Companionship and Chore Services paid for by the individual receiving care or their family.  For individuals without long-term care insurance or who do not qualify for income-based programs, paying privately for non-medical home care can allow an individual to continue to live independently without constant support of family members or friends.

The Connecticut Statewide Respite Care Program is a joint partnership between the Alzheimer’s Association Connecticut Chapter, the Area Agencies on Aging, and the Connecticut Department of Aging and Disability Services.

Family members caring for someone with Alzheimer's or another form of dementia at home can receive limited financial assistance. The program subsidizes the cost of services that can be used for a wide range of respite services available during the day or overnight.  In addition to providing relief to caregivers, the Connecticut Statewide Respite Care Program may also assist in developing a plan of care, and offer information and access to support caregivers.

The National Family Caregiver Support Program (NFCSP) is funded by the Administration for Community Living, and is operated in partnership with the State of Connecticut Department on Aging and the Connecticut Area Agencies on Aging. The Program funds a range of supports that assist family and informal caregivers to care for their loved ones at home for as long as possible.

NFCSP grantees provide five types of services: 1) information to caregivers about available services, 2) assistance to caregivers in gaining access to the services, 3) individual counseling, organization of support groups, and caregiver training, 4) respite care, and 5) supplemental services, on a limited basis.
