Self-Directed Care Made Easy!
The Community First Choice (CFC) program allows individuals who need hands-on assistance with Activities of Daily Living (ADLs) to access Personal Care Attendant (PCA) services and additional supports by self-directing their care. However, many CFC participants have difficulty navigating some aspects of self-directed care, so they may utilize the Support & Planning Coach to assist with Care Planning, developing their CFC budget, identifying and hiring caregivers, scheduling care, processing payroll, and handling disciplinary action.
Self-direction allows program participants to determine which services and supports will best allow them to remain independent and living within the community. This includes determining how to utilize their given budget to access the supports and services they need to live safe, happy lives within the community and avoid costly nursing home or assisted living care.
